We would like
to offer a brief summary of our program, philosophy and successes to date. Eagle Eye Farm Rehabilitation Center has been addressing the unique needs of the Traumatic Brain Injured survivor, as well as other "at-risk" populations, for approximately nine years. Our staff, many are professional educators, have been working successfully with "at-risk" populations for over twenty years. Our client-centered program addresses the needs of the TBI survivor, and other "at-risk" populations, presenting significant behavioral challenges. The educational, rather than medical model, approach to program design/implementation is flexible and constantly updated with information obtained through continuing education courses as well as the numerous conferences that staff attend. Program design focuses on "hands-on" learning where residents work on vocational projects while at the same time addressing identified academic and other needs. The residential site, located in a rural area of Vermont's Northeast Kingdom, offers residents an environment that is relatively free of stresses often associated with more urban areas. In addition to our main campus, located on a mountaintop, enhances security relative to residents that may be at-risk for elopement. A main farmhouse, three apartments, two "stand alone" homes as well as lake housing provide alternate living accommodations for residents as they progress in their respective programs. Community based activities are located approximately six miles from the residential site and provide access to church, shopping, banking, laundry, and various social activities. Program offerings include, but are not limited to, vocational opportunities, academic opportunities, employment opportunities, and recreation. Eagle Eye Farm Rehabilitation Center is a licensed Level IV Therapeutic Residential facility. Eagle Eye Farm Rehabilitation Center is also an approved Medicaid Provider. In addition to working with Medicaid patients we also collaborate with insurance companies, families, state agencies (Vocational Rehabilitation/ Mental Health) and the court system relative to program design for each resident. Residents have access to an excellent regional health center that provides on-site dental and health maintenance services. Dartmouth-Hitchcock Medical Center provides psychiatric interventions. Psychotherapy services are available locally as needed. Our client-centered program is focused on providing services in a non-institutional setting espousing a home-like atmosphere for both resident(s) and staff. To this end we make every attempt to maintain an average census of ten or less residents. Our lake facility, a large farmhouse with wood-shop, barn, etc., is designed to meet the needs of two residents. PROGRAM COST The Treatment Team, using the Medicaid Waiver as a guide, determines program costs. Medicaid and or Medicare usually cover client medical expenses.
